Client Assessment
Clients will be professionally evaluated to identify which level of care would benefit them most, and to understand the particular needs and concerns of each family. Many aspects are considered to make a successful and appropriate placement including but not limited to physical and mental health, activities of daily living, end of life issues, budget, and location.
Facility Screening
Facilities are company screened before being invited into the TSP, Inc. network of referrals. A Facility Profile is completed detailing the particular services, licenses and certificates for the different levels of care. Facility records are examined through the State Department of Social Services licensing office to substantiate compliance and to identify any operational or care deficiencies in the history of the facility.
To maintain compliance facilities must provide evidence that their staff has completed a Medication Training Program. For differing levels of care the staff must complete an Alzheimer’s and Dementia care training program including continuing education each year. A Hospice Waiver must also be granted by the State Department of Social Services, Community Care Licensing to accept, retain, or place residents on hospice care services.
